GUIDELINES FOR STONE PATIENTS

 

1-Use plenty of water, drinks, juices, fluids, liquids and every halal thing in drinking form
Drink water in adequate amount regularly in short spells. Boiled, filtered or mineral water
is better. Commonly, fifteen to thirty glasses per day are advisable, according to weather &
individual patient's requirements.
Drink first policy: Always drink plenty of water before going for out door activities, in sun
light, exercise, traveling, stressful sitting , before meals & soft drinks, after voiding &
before going to bed. In addition to plane water, use of fresh juices especially lemon &
orange and other drinks like lassi, milk shakes, sharbats ( bazoori, sandal, squashes,) and
hot drinks are also recommended.

 

2- Protect yourself from sun and hot environment.

 

3- Control your sugar, blood pressure and weight.

 

4- Keep yourself free from all tensions, worries, grieves, sorrows and other stress ful situations as much as possible.

 

5- FOOD SELECTION IS NOT VERY IMPORTANT BUT DO N'T OVER EAT
Take the balanced diet in adequate amount and don't restrict to any specific diet until advised by your doctor.
There fore the fowling food can be taken in a suitable quantities :-
Spanish , Tomatoes , Rice , Cauliflower , Green leavy salads and vegetables , beet roots , Strawberries , Cherries , Barriers , Beans and peas.
Milk and dairy items for instants Yogurt , Butter Milk , Custard , Sweets made from milk , Ice Cream etc.
Meat, beef , brain , joints , fish , eggs, lentils etc.
Tea , Coffee , Cola , chocolate , Dry Fruits etc.
Although the above food items have particles which help for stone formation like calcium, oxalates, uric acid etc but its definite proof is still lacking.

One must drink adequate water so that urine shouldn't pass late, is not concentrated, thick, deep colored or smelly, rather it should be frequently passed, thin, light colored, non smelly & in copious volume.. Modern research has proved that if water intake & urine out put is adequate, there are less chances of stone formation without any restriction of diet. But, if water intake & urine out put is in adequate,, stone formation is frequent even with any strict diet regime. Therefore, it is worth mentioning that drinking excess water is more important than selection of diet.
One must know the cause of his/her stone formation
The work up to know the cause of formation of stone is as important as treating the stone itself. Because, stone formation is a dynamic & continuous process. Stone forming factory ( kidneys) are still present, the raw material for forming the stone is also available (kidney status, blood, diet, water), and the stone forming environmental & genetic factors are also the same as before. If we do not work up to know the cause of stone formation and will not take the measures to treat these causes, stone will reform as before.
So follow up for a kidney stone patient is very significant to restrict the future reformation. The follow up should be by a qualified physician or urologist, who has a comprehensive command on the subject.
Regular follow up visits (3-6 months interval) are recommended.. The stone starts forming as a small particle and if detected at this early stage, it can be cured with medicines, If patient does n't come for follow up, this small particle will not be detected early & will continue growing up with time to reform stone again.
We, at our hospital, provide the follow up facilities also. So under the same roof top, patient is accessed, treated & followed up, cutting down the inconvenience of going to various places. We also keep the computerized record of every stone patient, which helps in better assessment & follow up & of patients.
Self recommended ultrasound is misguiding:
Some patients start evaluating their stone disease by self recommended ultrasonography from any doctor of their own choice, without knowing, whether he is a qualified or not and focus on the size of stone only. This illiterate approach is misguiding & confusing for the patient. It's not advisable for a patient to analyze his treatment progress on the basis of stone size only, mentioned in ultrasound report, without consulting his / her doctor.
a-The stone size may be shown as increased after ESWL, as the fragmented stone occupies more area and the ultrasound will give the shadow of larger area.
b-Majority of stones are not round, rather oval, irregular & branched staghorn type, having different measurements in different directions. Their exact size in one direction is not possible and ultrasound report usually mentions only one size, which does not reflect the true picture.
c-There are so many other important factors, to be considered by a qualified doctor or urologist apart from the size of the stones, about which the pt. is not commonly aware. For instance, how much damage is occurring in the kidney due to that specific stone? What role that stone is playing in the patient's disease? What could be the reason for the formation of that stone? how to prevent it from reformation again and many more.?
So our message is, do not self evaluate the progress of your case by mere ultrasonography. Always consult a qualified doctor or urologist, narrate your problem to him, he will be the proper person to advise the relevant investigations including ultrasound.
